I was privileged for 36 years to be trusted to provide retinal care in Northwest Ohio and in the third world. I retired from clinical practice on June 14, 2024.
If you need records, click on the link below and fill out the form. Records can be sent to the destination you specify. If you are a company requesting records, you will need to send a notarized SIGNED copy of the patient's contract with you proving your authorization to receive the records. Unfortunately, in this age of rampant fraud, this is the only way to protect patient privacy. The practice is closed, and there is no longer a business or billing office or in fact any office at all.
